help required, How to set up a contact form? in J 1.5

I'm trying to set up a contact form but I'm not sure were to start.  Been trying to figure it for the past 2hrs. I'm very new to Joomla and a step up setp guide would be great.  or some pointer to get me going.

The form would be some think like:
First Name:
Last Name:
Service: (pos with a drop down box)
Tel:
Email:
Web:
