Title: Image /video uploads - How can I do this

I want to introduce a section to my new site in development

I want to allow visitors to upload a picture or video while being able to add information also

I know that you tube can be intergrated into a site, but I was wanting to keep the look and feel of the site as per my design

Has anyone done this kinda thing so far (im sure you have) Can I have a link to view please
