SOLVED Menu Editor Not Always Updating

I am adding menu items in the menu manager and sometimes they are not updating in the menu editor. Also unpublished menu items are showing up in the menu editor. I've cleared the cache and reloaded the admin page to no avail. It worked once, then I added columns, move items to columns, then went back to the menu manager and added more items and now they won't show up in the menu editor and a unpublished item came back.

Please post your URL, Username and Password in Secure Tab and i will take a look and maybe figure out what the problem is.
